چکیده

An 81-year-old man with retinitis pigmentosa had severe, slowly progressive visual loss that began at age 20. Intermittent nystagmus was observed for 1 decade. Examination revealed severe retinal atrophy and periodic alternating nystagmus (PAN) that occurred only in darkness (video on the Neurology® Web site at www.neurology.org). PAN refers to horizontal nystagmus that periodically reverses direction. It is caused by instability of brainstem velocity storage mechanisms that regulate rotationally induced nystagmus, with intact vestibular “repair mechanisms” that are calibrated by visual input. PAN may emerge with impairment of central visual stabilization mechanisms or deficient visual input. This case demonstrates that the presence of PAN may correlate with ambient lighting and maintenance of visual fixation.

Instabilities in Eye Movement Control: A Model of Periodic Alternating Nystagmus

Nystagmus is a pattern of eye movement characterized by smooth rotations of the eye in one direction and rapid rotations in the opposite direction that reset eye position. Periodic alternating nystagmus (PAN) is a form of uncontrollable nystagmus that has been described as an unstable but amplitude-limited oscillation. PAN has been observed previously only in subjects with vestibulo-cerebellar ...

Induction of periodic alternating nystagmus in intact goldfish by sinusoidal rotation.

During movement, the vestibulo-ocular reflex (VOR) normally maintains retinal image stability by making slow-phase eye rotations that counterbalance head rotations. These eye rotations are nystagmic when the on-going slow-phases are interrupted by fast-phase eye rotations that reset eye position. Periodic alternating nystagmus (PAN) is an eye-movement disorder characterized by uncontrollable ny...

Analysis of a case of periodic alternating nystagmus.

A patient is described in whom there was periodic deviation of the eyes in darkness associated with alternating nystagmus. An exactly similar situation occurs in normal man subjected to a pendular stimulus. It is proposed that in this patient there was alternating activity of the vestibular nuclei on either side of the brain-stem as a result of a removal of cerebellar inhibition, and that this ...

Teaching video neuroimages: see-saw nystagmus.

A 43-year-old woman experienced a worsening of her usual headache. As a right hemifacial hypoesthesia had appeared, the patient underwent a brain MRI that revealed a giant cavernoma localized at the left meso-diencephalic region (figure, A and B). After the operation, an involuntary ocular oscillation known as see-saw nystagmus developed (video on the Neurology® Web site at www.neurology.org). ...

Periodic alternating nystagmus in humans with albinism.

PURPOSE To quantify the spatial and temporal nature of congenital periodic alternating nystagmus (PAN) and to test the hypothesis that PAN results from a temporal shift in the null zone. METHODS Twenty-five subjects with oculocutaneous albinism (16 tyrosinase negative and 9 tyrosinase positive) and 7 with ocular albinism (5 x-linked and 2 autosomal recessive) participated in the study. Using ...
